I am sorry to hear of your health problems! That can't be fun.
Also, you sound like you've risen above your family background by your bootstraps and it's good of you to extend a hand to other members that know there has to be something better out there for them.
In my case, I do love my family, for the most part, but I'm very much a black sheep, and have managed to dodge all major functions and responsibilities by moving to the Midwest. ;->
